So what's the difference between Teen Belle and Teen Josie?

I mean, I know Belle is "younger" than Josie, but is one supposed to be more realistic? One more toony? Belle seems to have a bigger head?

...I wouldn't say so much "toon" as perhaps "stylised". To me toon refers to characters like Sadie, Sam, Dolly (Rendo), Mavka, Hitomi, the original Girl, etc..

I have both Belle and Josie and in looking at the parameters tab, Josie's body and head shape is grouped in 'Real World" while Belle's are grouped under "Stylised". I find this a bit odd as most people feel the opposite is true.

Just after I installed Belle I did an experiment using her as the basis for my Leela character and found needed to do far less shaping, scaling and morphing than even with YT5 Julie, and (at least to me), she looks more like a "Real World" teen than ever..

I've also used Josie as the basis for one of Leela's friends (not the elf girl Tracey) and she looks very believable on as well. Have yet to try a mix of the two.

Don't be deceived my the head. A young organism has different proportions than a developed specimen. Humans start with heads that are relatively big when compared to the rest of the body. When they grow, other parts of the body become bigger or longer, whereas the growth of the head is not so remarkable. Compare a newborn to an adult: in the first case, the rule 1/7 doesn't apply. So Belle, being noticeably younger than Josie (and still growing), has a bigger head compared to the body.

Belle is a young teen, Josie is an idealised look of a very "developed" teen, more approaching her twenties than anything. Think Belle - starting junior high school; Josie - taking her matriculation exam.

It is also reflected by the respective bundle scheme. Josie's has skimpwear, Belle's does not.

...I've worked extensively with Zev0's Growing Up morphs and yes, in the younger morphs, the head to body proportion is larger.

In my Leela experiment, I used a small amount of the Growing Up "youth" morph to get the proper character height (5' 1") and physique rather than just scale her down as the morph also changes the overall proportions (including the head) as well. I also added in a bit of the "youth posture" morph to eliminate some of the "swayback" stance Daz female characters tend to have.
