I always use Decals for things like that (You could use a geometry shell). Of course you could manually add rimstones in 3d space, but i use a decal with a transparency map. (You could edit the texture of a existing jacket in Gimp or Photoshop to add the stitching).

Thats not going to be easy. I think, i would try it with a geometry shell and see if you find a good Rimstone texture with a Displacement map, that could add the 3d effect. But the problem is, that they would stretch and bend and distort while you pose the character... I have no Idea at the moment, of how that could be achieved, besides a loooooooooong time of trying to create a map to make every single stone "rigid" so, it does not distort, as you could do with belt buckles. But to be honest, i never really understood those tutorials, and don't know how to explain that easily.

Ah. that could be done with a bump map. There are free tools to convert "normal" pictures to Bump or Displacement maps. But i also know that only in theory, i am still learning to use all those tools, that why it takes so long for my new series of free clothing to come out. I am still not happy with the results... And didn't even start to rig and correct the maps...
