How do decals work

I'd like to apply a decal to my characters hand. I have the node created and positioned, I just can't remember what to do next. It's been awhile and I can't find the thread Can anyone help me with the next step so I can apply the shader?

Okay, I think I have it figured out but its not applying the shader to the entire hand, just the top surface. I remember there being a way to apply it front and back but can't remember how. I have both of them slected in Surfaces.
