No matter what I do to alter the genesis body shape, the breasts shows through Mad Nurse for Genesis Female. Applying Smoothing modifier to dress doesn't do anything either.

Are you sure the clothes are fitted to Genesis?
I've had similar issues with freebies I've picked up along my travels. Changing the smoothing modifier to 10 and setting the collision iterations to 3 fixed any poke through I had. Maybe that could help with the issue you're having?
Here's a screen capture of the trouble.
It seems to load with the Collision Iterations set to zero. Select the dress, then Parameters tab -> Mesh Smoothing and increase the Collision Iterations.

I removed smoothing modifier entirely and didn't get that problem.
Don't worry just realised it was already fixed.
You can also hide the torso on Genesis, opacity = 0 on the surfaces tab
I'm not too fond of the posing problems with this outfit. The cape in particular. I haven't been able to find the readme. I may return this.
I wanted to add something to the thread. Both versions of the dress work well in Poser (both G and G2). I did get an error message when unzipping the G2 Poser file, but this does not seem to have affected how the dress installs into Poser, or using the dress in Poser (PP2014). Just letting you know that some folks might get an error message unzipping the G2 Poser file.
Thanks for a great product - this will be very useful as a typical 50's dress - when used with other textures and MATs.
Is the cape even usable with arms up poses?

Instructions on how to pose the cape
The Cape for Mad Nurse for Genesis was modeled with the figure’s arms in the down position instead of the extended position (zero pose). This was to prevent conflict with the shoulders, forearms, chest and abdomen groups and to avoid mesh distortion when the arms are down.
After loading the rest of the Mad Nurse clothing, position the figure’s shoulders down. With Genesis selected, load the Cape.
DAZ Studio and Poser have different ways for setting clothing, especially clothing of this type. Poser users should use the CollarAdjPoser morph (under the Adjustment group).
Pose Genesis to the desired position. Using the provided morphs and body handles on the Cape, move the Arms area until you achieve the results you want.
Note: The Cape is not well suited to extreme poses due to its shape and rigging.

That would probably explain why I am having problems getting the cape to work as a Poser dynamic clothing item, having made it into an OBJ via Carrara.
Yes, the figure should be inside the clothing when you start to drape.
Not sure but sounds like it might be.
I did make sure the instructions are in both the readme's. If you have any other questions on the cape let me know if i don't know the answer i will find out from the artist.

2mm push modifier often fixes those type of problems without disturbing the look of the outfit too much. But I find if it's just one part of the body causing the trouble it's easiest to just make a morph that only targets that area and apply it when needed. This helps prevent the outfit turning into liquid latex and enveloping other body parts that come too close to it like it was Peter Parker's symbiot suit (which is the problem with a smoothing modifier).
I still haven't found the readme. I installed it via DIM. I checked 2 different readme folders. I guess I haven't found the right one yet.
