How to put holes in shirts?

I'm trying to make a heart shaped hole in a shirt, so that part of the shirt isn't there, just the figure's skin. I have UV Maps and Photoshop and I know how to change colors, but I can't wrap my head around this. Can anyone help? Thank you.

That can be done fairly easily, lovessuperheroine. The trick is to create a new black and white image from the existing texture map that is entirely white except for the heart-shaped cutout desired, with the latter being pure black. That map would then be plugged into the cutout opacity channel in Iray or the opacity channel in 3Delight. The black (heart) area will be transparent, and the rest show normally.
