Help on how to cut holes in textures

Hi everyone
I'm looking for a tutorial which can help explain how to create a new texture for an outfit, but with gaps in it. For instance the regular textures for a top might include a fully covered back, and I'd like to know how to add a hole in the back material instead.
If a tutorial doesn't exist, I'd still like to hear someone describe the basics of what I should be doing as a starting point.

You need an opacity map, white where the surface should be solid and black where the hole should be. That gets applied to the Opacity or Cutour Opacity (depending on shader) property.
