I wonder if you could help me with this hair poke through situation. What's causing it and how can I avoid it?

Is your character very far away from the 0,0,0 origin? I have seen transparency go all wonky like that if you are far from the origin and render.
Its the hairs skull cap poking through, You need to use the Hairs Adj Morph - Head Size to get it to fit correctly

Very very far no but she isn't close to the origin either.
I did a test with posing the character at 0,0,0, and oddly enough it worked, the hair poke through disappeared. This is really weird.
