I saved item as prop - the file name has "Follower?" tag

Why it has question mark on the tag?

What is the difference between saving as wearable and as figure/prop?

Because it has no 'sub Content Type', give it one before saving asset. In general, Figure / Prop is an asset, or in other word, a Content Type while a Wearable could be a Category or a Preset depending on the funciton you use...
