Do I understand the you want the primatic diffraction effect on the skin that sometimes can be seen?
If so, I think this can be achieved in Iray by turning on Caustics in render preferences and using the correct water shaders. I would guess that a great deal of experimenting with light sources, strengths, and placements will be required.
All a guess mind you (but a probable future experiemnt).
Yes, it will...and correspondingly increase the render time. Iray will do reflective caustics as well as refractive (the light playing through water or glass).
